Good Health Direct Primary Care and Wellness Clinic is owned by Consuelo and Michael Bartyzal. We created this practice to provide a better healthcare experience—one that is personal, preventive, and focused on helping you achieve your best health.
CONSUELO BARTYZAL, FNP-C
Consuelo was born and raised in Albuquerque, New Mexico. She graduated with Honors with her Bachelor of Science in Nursing from the University of New Mexico in 2010 and moved to the Rogue Valley in 2011.
She has spent the last 15 years working in a large health system in a variety of roles, including general medicine, critical care, and hospice.
In 2019, she graduated with Honors with her Master of Science in Nursing as a Family Nurse Practitioner. For the past 6 years, she has served as a primary care provider in a busy family medicine clinic at the same large health system.
Consuelo decided to leave the large health system and open Good Health Direct Primary Care and Wellness Clinic because she wanted to be able to dedicate more time to her patients and provide truly individualized care.
MICHAEL BARTYZAL
Michael was born and raised in the Rogue Valley. He has a background in freight transportation and sales.
At Good Health Direct Primary Care and Wellness Clinic, you will likely see Michael at the front desk, helping to create a welcoming and smooth experience for every patient.
OUR WHY
In today's fee-for-service healthcare, most providers have over 1,500 patients each. Even when a provider really wants to spend more time with their patients, it really is not possible.
Consuelo truly believes in preventative medicine and is dedicated to helping patients better their health and hopefully help give them a better quality of life.
By practicing Direct Primary Care, we are able to focus on what matters most—building lasting relationships, spending time with our patients, and helping you feel your best.
We are honored to care for individuals and families through every stage of life.
OUR FAMILY
Consuelo and Michael married in 2014 and have two wonderful children. As a family, we love fishing, spending time on our boat, and enjoying all that the Rogue Valley has to offer. We also have a mini Goldendoodle named Ruffles who is like our third child!

Schedule a 15-minute visit so we can meet, learn more about you, and see if our approach to care is the right fit for you and your family.
Step 2: Establish Care Appointment
Once you decide to join, you will have a 1.5 hour (90-minute) establish care appointment so we can review your past medical history and discuss your health care goals.
