A simpler, more personal way to receive primary care in the Medford area.
Direct Primary Care (DPC) is a membership-based approach to primary care. Instead of paying for every appointment, message, or phone call, members pay one predictable monthly fee for ongoing access to their provider.
• Unlimited primary care visits with your provider
• In-person office appointments
• Telephone and video visits when appropriate
• Secure text, email, or portal communication
• Preventive care, acute care, and chronic-condition management
• Minor office procedures, including services such as sutures and joint injections
For covered procedures, there is no additional provider fee. Members are responsible only for the clinic's at-cost price of any medication or disposable supplies needed to complete the procedure. Watch the 3 minute video here ->

Good Health Direct Primary Care and Wellness Clinic is owned by Consuelo and Michael Bartyzal. We created this practice to provide a better healthcare experience—one that is personal, preventive, and focused on helping you achieve your best health.
In today's fee-for-service healthcare, most providers have over 1,500 patients each. Even when a provider really wants to spend more time with their patients, it really is not possible.
Consuelo truly believes in preventative medicine and is dedicated to helping patients better their health and hopefully help give them a better quality of life.
We are honored to care for individuals and families through every stage of life.

Membership Terms
• 12-month commitment: Membership is billed month to month, with an initial 12-month
commitment.
• Cancellation: Membership may be canceled with one month's written notice.
• Early cancellation: A $100 cancellation fee per member applies if membership is canceled
before completion of the initial 12-month term.
• Enrollment fee: A one-time, nonrefundable $100 enrollment fee is due at signup.
Beyond Primary Care: Additional Wellness Services
Good Health Direct Primary Care and Wellness also offers additional services designed to support health, pain management, mobility, recovery, and quality of life. These services are separate from the DPC membership, but members receive discounted pricing.
