Plan for approximately 30-45 minutes. The visit includes review of your medical history, medication list, vision and hearing screening, vital signs, urinalysis as required for the DOT examination, and a physical examination based on FMCSA standards. Certification length depends on the driver's medical history and exam
findings.
Before Your Appointment
Please come prepared. Bringing the right paperwork and medical information can help prevent delays in certification.
- Print and complete your driver section before arriving. DOT medical forms can be downloaded from the
Federal Motor Carrier Safety Administration (FMCSA). - Bring your glasses or contact lenses. If you use corrective lenses to drive, bring and wear them for your
examination. - Bring your hearing aids. If hearing aids are needed to meet the hearing standard, please bring them to the
appointment. - Bring an up-to-date medication list. Include prescription medications, over-the-counter medications, and the names of the clinicians who prescribe them.
- Bring relevant specialist or testing information when appropriate. Most drivers do not need recent laboratory results. Certain chronic conditions may require additional information to support certification.

Do I need Lab Results?
Usually, no. Recent laboratory results are not required for most routine DOT physicals. However, if you have a chronic medical condition that may affect DOT certification - such as diabetes, kidney disease, heart disease, significant anemia, or another condition requiring ongoing monitoring - bringing recent results can help the Medical Examiner understand how well the condition is controlled.
Additional Forms you may need
Not every driver needs these forms. They are used when a specific medical condition, vision issue, or medication requires additional documentation.
Important for non-members: If you are not a Good Health DPC member and an additional FMCSA
form is needed, please have the appropriate treating clinician, prescribing clinician, optometrist, or
ophthalmologist complete it before your DOT appointment. Because we may not have access to your
longitudinal medical records, arriving with completed supporting documentation can prevent delays.
MCSA-5870 - Insulin-Treated Diabetes Mellitus (ITDM) Assessment Form
For drivers treated with insulin. The driver's Treating Clinician completes this form to document that the individual has a stable insulin regimen and properly controlled diabetes. The completed form must be provided to the Certified Medical Examiner within 45 days of completion by the Treating Clinician.
MCSA-5871 - Vision Evaluation Report
For certain drivers who do not meet the standard vision requirement in the worse eye for distant visual acuity, field of vision, or both. The report must be completed, signed, and dated by an ophthalmologist or optometrist before the DOT physical. The Medical Examiner examination must begin no more than 45 days after the eye-
care professional signs and dates the form.
MCSA-5872 - Non-Insulin-Treated Diabetes Mellitus Assessment Form
An optional form that provides the Medical Examiner with additional information about the diagnosis,
monitoring, treatment, and stability of non-insulin-treated diabetes. It may be requested when more
information is needed to determine whether the driver meets the physical qualification standards.
MCSA-5895 - 391.41 CMV Driver Medication Form (Optional)
An optional form that allows a prescribing clinician to provide additional information about medications and
the condition being treated. A Medical Examiner may request it when medication-related information is
needed to determine whether a driver is medically qualified under 49 CFR 391.41.
A Few Helpful Reminders
- DOT certification is based on FMCSA standards and your individual medical history. Scheduling an examination does not guarantee certification.
- If you have a significant medical condition, recent hospitalization, surgery, sleep apnea treatment, cardiovascular history, seizure history, or other condition that may affect safe driving, bring relevant records and specialist documentation when available.
- If additional records or forms are needed, certification may need to be delayed until the Medical Examiner receives enough information to make a determination.
